Pillars of Dermatology and Arise Combined Meeting

The link between obesity and skin health: I attended a meeting in Melbourne on 22 and 23 February 2025 at the Grand Hyatt. This meeting was attended by dermatologist and rheumatologist discussing various aspects of combined meetings between these two specialties noting that we often use the same drug to treat the skin and joints.

One of the interesting talks was on obesity by Associate Professor Samantha Hocking. Obesity can affect our overall health and in particular heart and diabetes. Obesity can also affect the skin and can lead to irreversible conditions such as chronic lymphoedema. This is despite once there is significant weight loss there can be significant redundant skin particularly on the lower limbs which is not improved with weight reduction. Other skin changes that can be seen in obesity includes pseudoacanthosis nigracans whereby there is velvety hyperpigmentation on the sides of the neck and in the armpits. Skin tags can also PE as a result of obesity. Rashes in folds of skin can lead to candidal intertrigo. Pressure ulcers are more common in obese patients as well as plantar hyperkeratosis of the soles of the feet. Lymphoedema can also lead to cellulitis of the skin. Hyperandrogenization such as excessive facial hair is more common in obese patients.

Loss of weight leads to improvement in all health parameters such as reduction in waist circumference, blood pressure, triglycerides, fasting insulin, return of blood sugar level to normal, HDL cholesterol.

GLP-1 receptor agonists have anti-inflammatory effects leading to improvements in joints and skin. Arthritis tends to improve as well psoriasis.

GLP-1 receptor agonist have also been showing promising improvement in hidradenitis suppurativa.

GLP-1 receptor agonist have also shown to improve NASH or metabolic dysfunction associated steatohepatitis.
